Reablement (Hospital to home)
This is short term care at home, to aid the full or partial recovery after hospital discharge. Reablement services help people to retain or regain their skills and confidence so they can learn to manage again after a period of illness. Freeman’s Choice provides these services in the comfort of your own home.
End of Life Care
It can be reassuring to know that at the last stage of life, you are surrounded by your family and loved ones at your home, a place where there are so many memories. At Freeman’s Choice, we work with other multidisciplinary teams like the Palliative team, the Doctors and District Nurses to ensure that at the last stage of your life, you are comfortable and pain-free.
Respite Care
Everyone needs a break once in a while and that is why we provide respite care. A short-term passage of care, often for one to two weeks or many more. These can be either planned or an emergency and at Freeman’s Choice, we are always ready to provide you with that support anytime-any day.
Live-in Care
With Freeman’s Choice, be rest assured that the care we provide is compared to none. Our care professionals are chosen to be a companion to your loved ones to live in their homes and support them with their everyday activities. A live-in caregiver is an available day and night for people who need assistance 24hours of a day.
